Student Registration
All non-English birth certificates must be officially translated.
McKinney and Refugee Students: Please refer to the Student Status and Registration Requirements document.
Parents who are unable to provide a Certified Copy of a Birth Certificate should use the affidavit along with other reliable proof. The affidavit must be signed, notarized, and accompanied by other reliable proof. Currency Exchanges, banks, and some local libraries provide notarization.
Non-MV/Refugee students who start registration without a Birth Certificate or the signed Affidavit may start school immediately. This is separate from the residency and medical requirements.
If a birth certificate (or signed affidavit) is not provided, immediately prepare and email the Missing Birth Certificate Letter - 30 days. (30 calendar days, not school days). If a Birth Certificate or the signed/notarized affidavit and additional proof are not presented within 30 days, prepare and email the Missing Birth Certificate Letter - 10 Days. Work with your administrator, who will notify Dr. Chung and the local law enforcement.

Housing Deviations
Letter of Residency from Landlord in Lieu of Lease 7:60-AP2.E1
Collect this document along with two additional items from category II from the person enrolling the student. The expiration date must be entered as June 30 of the CURRENT school year.

Living with a District Relative: English, Spanish. The expiration date must be entered as June 30 of the CURRENT school year.
Leases: All leases must have the expiration date entered in PS.
Evidence of Non-Parent’s Custody, Control, and Responsibility of a Student 7:60-AP2.E3
This form establishes a child’s residency in the School District when the child is not living with a natural or adoptive parent. It must be completed by the individual who has assumed custody. This is not used for DCFS placement.
The Request for Student Records must be within 14 calendar days after enrolling a transfer student.
Registration MV, Refugee Families and DCFS Placement
MV Students - Status Change & Residency
If the school Social Worker removes a family from MV status, the family would then be required to submit proof of residency. Depending on when their status changes, the due date for compliance will change. Families will not be excluded from D63 mid-year. The information in this attached letter can be sent via email to the family. Please reach out to Debbie if you have any questions.
Refugee Status - Interesting Information
Indefinite Status with a Path to Citizenship
- Right to Remain: Asylees can remain in the U.S. indefinitely without fear of deportation, as their asylee status does not expire.
- Green Card: One year after being granted asylum, you can apply for a Green Card (lawful permanent residence).
- Citizenship: After holding a Green Card for five years, you become eligible to apply for U.S. citizenship.
