Important Information re: Physical and Immunization Appointments
The State of Illinois requires that parents present to the school a current physical and immunization record for a child entering kindergarten, 6th grade, or new to a school. This information is required to be provided to East Maine 63 (EMSD 63) by the first day of school, August 20, 2020.
However, due to the current situation regarding the Coronavirus, we understand it may be difficult to schedule an appointment with your child's healthcare provider.
For this reason, if you are unable to provide the required health information by the first day of school, EMSD 63 will accept a letter from your child's healthcare provider that documents a scheduled appointment for a physical exam on or before October 15, 2020.
The letter for the upcoming appointment must be written on the healthcare provider's office letterhead and include the appointment date and time and it must be signed by the physician, nurse practitioner, or physician assistant. The letter for the appointment must be presented to the health office on or before the first day of school. Failure to provide the required health information or note that documents a doctor's appointment will result in your child being excluded from school.
Completed health forms must be turned into the school health office the next school day after the scheduled appointment in order for your child to remain in school. Students who do not have the required health form and immunization record turned in by October 15, 2020, will be denied the privilege of attending until the obligation is met.
