Important Information re: Physical and Immunization Appointments
The State of Illinois requires that parents present to the school a current physical and immunization record for a child entering kindergarten, 6th grade, or new to a school. This information is generally required to be provided to East Maine 63 (EMSD 63) by the first day of school.
However, due to the current situation regarding COVID-19, the Illinois State Board of Education issued new guidelines as of October 12, 2020.
- Students who are not compliant with state physical and immunization requirements by October 15, 2020 will still be able to participate in 63 Online, our remote learning program.
- However, students who have registered for 63 Hybrid, our hybrid in-person learning program, must have required documents on file before they can return to our buildings. If students do not have these required documents on file by the scheduled return date for their grade, they will continue to participate in 63 Online until the documents have been submitted. Once the document are submitted, they may attend 63 Hybrid.
Completed health forms must be turned into the school health office. If you have questions, please contact the nurse at your child’s school.
