Attendance/Illness

Check your child for illness before sending him/her to school.  Please do not send your child to school if he/she has had a fever over 100.00, vomited or had diarrhea within the last 24 hours.  You must keep your child home for an additional full day after he/she has had a fever, diarrhea or has vomited.  Please call the Attendance Line each day your child will be absent from school and give a detailed reason.  Please report all illnesses with a fever.

Emergency Information, Health Information

It is very important to contact the nurse in the Health Office in your school regarding any health problems that your child has or develops during the year.  It is also very important that you can be reached immediately in the event that your child becomes ill or injured during the school day.  Please make sure all phone numbers on the registration form are correct and notify the Washington School Main Office, 847.965.4780, of any changes. 

Medication 

School District 63 policy requires a written order from your doctor as well as written permission from the parent before any medication can be administered whether or not this is over-the-counter or prescription medication.  All medication must be in a properly labeled container from the pharmacy and must include the child’s name, physician’s name, name of the medication, dosage and time at which it must be given.  Medication will be administered only if it is brought to school by an adult.  We do not allow children to carry medication to and from school except for inhalers with a doctor’s order. 

Medication brought to school that does not meet District 63 requirements will not be administered. 

Dressing for the Weather

Please make sure that your child is dressed appropriately for the weather.  All children go outside for recess.  When it is cold outside please dress them in a coat, hat and gloves or mittens.  It your child is younger you may want to keep a change of clothes in their locker at school for falls into puddles/mud or toileting accidents. 

Requirements for Kindergarten students:

A current and completed Physical Exam form, with immunization records, Vision and Dental Exam forms, as required by the Illinois School Code. 

Requirements for 2nd grade students:

A current and completed Dental Exam form, as required by the Illinois School Code. 

Requirements for 6th grade students: 

A current and completed Physical Exam form, with immunization records and a Dental Exam form, as required by the Illinois School Code.
