Special Note Concerning the Tdap Immunization: 

A Tdap immunization is now required for all students entering 6th thru 12th grades in Illinois by the Illinois Department of Public Health.

 What is Tdap?

Tdap is a vaccine licensed and recommended to protect pre-teens, adolescents and adults against tetanus, diphtheria and pertussis (whooping cough). Tdap is licensed for routine use on or after the 10th birthday.

This fall, students entering sixth grade will be required to provide proof of a Tdap vaccination along with a physical using the new State of Illinois Certificate of Child Health Examination  form dated 2012.  The Tdap vaccine and date (month, day, year) administered must be on the form.
